Payment Policy

For Aetna clients:

Currently, Milkin' Melanin Lactation Services is in-network with Aetna. Please be aware that Aetna needs to be your PRIMARY insurance with a plan that offers lactation consultations in order for us to be able to submit claims for approval. If Aetna is NOT your primary insurance and we are not aware of that, we will have to bill you for the visit and send you a super bill for you to submit for possible reimbursement.

We require a credit card on file as a co-pay or deductible may be required by your insurance company.

Co-pays are typically confirmed once the claim has been processed by the insurance company which can take several weeks following the date of service. An Explanation of Benefits will determine the amount that needs to be paid by you (the patient) which will be charged to the card on file. We verify your benefits as a courtesy but it is your responsibility to reach out to your insurance carrier to confirm coverage.

Self-pay (out-of-network) clients:

We will provide you with a superbill suitable for you to submit to your insurance. The superbill (which will also serve as a payment receipt) will be coded appropriately to the level of service provided during the visit. You agree to pay Milkin’ Melanin Lactation Services at the time of the visit (debit/credit card, or FSA/HSA). You agree to pay the full cost of services per visit. You will not be refunded for the cost of services rendered. You understand that you must pay a $100 deposit upfront.

Using Stripe: Payments may be made electronically using a credit card or fund transfer. Milkin’ Melanin Lactation Services uses Stripe to process payments. Stripe meets the high standards of HIPAA and the banking industry for security and privacy with regard to financial transactions. However, Stripe may send, automatically or per your request, email or text message receipts that reveal personal health information such as the date and type of lactation visit. If you are not comfortable with this, payments may be made via PayPal instead. Please reach out to your lactation provider directly for PayPal information.

Cancellation/Rescheduling Policy: You understand that you are responsible for all charges associated with this visit. If you cancel within less than 24 hours' notice, the credit card on file will be charged $100. Deposits for self-pay visits can be used to reschedule your appointment. You will be able to reschedule up to four hours before your scheduled visit. Please reach out to Danielle directly at (203) 769-9312 if you are unable to reschedule on your own.

Travel Fees for ALL clients requesting home visits:

Starting January 14, 2023, a travel fee will be required for all home visits.

The fee schedule is as follows

• $40 within 20 minutes of New Haven.

• $70 for distances more than 20 minutes away from New Haven.

• $100 for distances 45 minutes to 1 hour away from New Haven.

Travel fees will be applied to your invoice once you schedule your appointment and will be charged to the card on file on the day of your appointment. This fee is not covered by insurance.

** Please note that home visits are offered on a very limited basis. Follow-ups will be offered in-office or virtual.
